Romantism And Rationalism Romanticism began in the mid-18th century and reached its height in the 19th century. The Romantic literature of the nineteenth century holds in its topics the ideals of the time period, concentrating on emotion, nature, and the expression of "nothing." The Romantic era was one that focused on the commonality of humankind and, while using emotion and nature; the poets and their works shed light on people's universal natures. Romanticism as a movement declined in the late 19th century and early 20th century with the growing dominance of Realism in the literature and the rapid advancement of science and technology. However, Romanticism was very impressionative on most individuals during its time. Rationalism or Realism was erected during the mid 19th century. Realism are ideas that are brought up in philosophical thinking. The realistic movement of the late 19th century saw authors accurately depict life and it's problems. Realists attempted to give a comprehensive picture of modern life by presenting the entire picture. They did not try to give one view of life but instead attempted to show the different classes, manners, and stratification of life. The Rationalist recognizes that they must master their own destiny, using their unique powers of reason and the scientific method to solve problems. Such authors that represent these two eras are Ralph Waldo Emerson, Henry David Thoreau, William Wordsworth and Charles Darwin. Romantics believed that one needed to understand nature to understand oneself. In other words, only through nature could one discover who they are. Emerson shows this in his writing called "Nature". In the exert "...man beholds somewhat as beautiful as his own nature." This depicts Emerson's feelings toward nature; view nature as you view yourself. If one views nature as caring and compassionate, one will also see themselves as caring and compassionate. Similarly if you understand nature you will know yourself better. As one gains wisdom from nature, one begins to realize that understanding is a gate way to the divine as well as to oneself. Other writers also agreed with this notion of nature. In the essay "Walden" by Thoreau, Thoreau had left society to move into a shelter outside of his town. By living on only the necessities he lived his life as simple as he could, thus finding the divine within himself. By being separate from society and being one with yourself are the only ways one can find the divine. Thoreau felt by doing this society would have a harder time to mold him into what it wanted him to think. Thoreau left a life of luxury for "voluntary poverty". Even though he was "poorer in his outward riches" he was wealthy in his "inward riches". A good number of romantic views of Nature suggested using Nature as ones tool to learn. This is evident in William Wordsworth's poem "The Tables Turned. In the poem "The Tables Turned" Wordsworth states to "quit your books [for it is] a dull and endless strife[;] enough of Science; close up those barren leaves." Wordsworth believed piece that books were useless to learn from. He believed that we should "Let Nature be [our] Teacher [for it]...may teach you more of man [and] moral good and evil[, more] than all the sages can." Wordsworth agreed with the previous notion that to understand the divine and oneself, they must first start with understanding Nature. This View of studying Nature is taken one step further by Charles Darwin. Perhaps the most appealing quality of Darwin's work was that it accounted for phenomenon in a purely naturalistic manner. It was the most scientific explanation yet, completely removing the supernatural explanation, and setting him apart from the theorists before him. The major unsettled scientific question of Darwin's Theory was be in regards to natural selection as the mechanism for change, which became the issue among the general public as well. Hey You!

Hey You! Hey You! Hey You! By Maeve Maddox Puleen Patel wonders about the appropriateness of hey as a greeting: I see most people online and offline address each other more and more by saying Hey David and so on. Is this correct? Is this a new thing? I always remembered addressing (and being addressed) as Hi David or Hi Puleen. When I was in school, many years ago, my English teachers objected to both hi and hey as rude utterances. However, both have been in the language for a very long time. Most dictionaries define hey as â€Å"an exclamation to attract attention.† Ditto for hi. Hey is often used as a nonsense word in song lyrics, as in the chorus of Simon and Garfunkel’s Mrs. Robinson. There’s a Jennifer Love Hewitt song called Hey Everybody. Earlier still, Shakespeare used hey in the refrain of a song in Much Ado About Nothing: . . . be you blithe and bonny; Converting all your sounds of woe Into. Hey nonny, nonny. Hey and hi can’t claim to have an etymology like a â€Å"real† word. Both seem to derive from the sound of a grunt, like Roman eho, Greek eia, and German hei. The OED notes that used as â€Å"a word of greeting,† hi is chiefly North American. According to the Online Etymology Dictionary, the first recorded use (1862) of hi as a greeting was to the speech of a Kansas Indian. This use of hi by an Indian brings to mind the â€Å"how!† so often heard in old cowboy movies. Again, the OEtyD: how: Native American greeting, Siouxan (cf. Dakota hao, Omaha hau); first recorded 1817 in Eng, but noted early 17c. by Fr. missionary Jean de Brebeuf among Hurons as an expression of approval (1636). As to which to use as an informal greeting, it’s a personal choice. I read a comment by someone who prefers hey to hi because â€Å"it sounds more casual.† To some speakers, however, hi sounds friendly, but hey sounds rude. Shape Memory Alloys Manufacturing Processes Smart materials have been one of the fastest growing materials needed for medical device manufacturing. Smart materials, according to the McGraw-Hill Dictionary of Scientific Technical Terms, are defined as Materials that can significantly change their mechanical properties (such as shape, stiffness, and viscosity), or their thermal, optical, or electromagnetic properties, in a predictable or controllable manner in response to their environment. It is this property of changing according to its material that makes smart materials very valuable in manufacturing today. Perhaps one of the most useful smart materials comes in the form of memory shape alloys, specifically nitinol. Memory shape alloys have many applications in medical devices used today. They are highly prized for their exceptional superelasticity, their shape memory, their good resistance to fatigue and wear, and their relatively good biocompatibility. This makes them the perfect candidate for many in-vivo medical devices . Origin The shape-memory effect was first observed in copper-zinc and copper-tin alloys by Greninger and Mooradian in 1938, but it was only in the early 1960s that Buehler and his colleagues discovered and patented nitinol, a nickel-titanium alloy created in the Naval Ordnance Laboratory (NOL). This lab was formerly located in White Oak, Maryland and was the site of considerable work that has had useful impact upon world technology. The White Oak site of NOL has now been taken over by the Food and Drug Administration but has still left its legacy in the name nitinol (nickel + titanium + NOL- the initials of the Naval Ordinance Laboratory) (Gautam, et al. 2008). Their smart metal alloy, however, is 55% nickel by weight and may thus have allergic, toxic, or carcinogenic effects. For short-term use, in-vitro and clinical data strongly support nitinol as a safe biomaterial which is at least as good as stainless steel or titanium alloys also available to designers. Medical Applications of Shape Memory Alloys Muscles are the power of the body, used to turn energy into movement and motion. Shape memory alloys can be used to in their solid-state phase to make devices from muscle wires. Applications of shape memory alloys in the medical field are numerous. Their flexibility at one temperature and one way shape memory effect when heated to their transformation temperature make these alloys key materials for various medical methods. The inability of shape memory materials to combine to other metals requires some adaptation to be developed. A common material for this is nickel-titanium. Nickel-titanium has an excellent torque transfer characteristic which is just one of the many reasons this material is used for fabricating medical equipment (Yoshida, et al. 2010). A few notable applications are catheters, medical guide wires, bone plates and stents. Bone plates comprised of shape memory alloys, assist in repairing broken bones by making use of the bodys natural temperature to contract and maintain pressure for proper healing. (Georgia Inst. Of Tech, 2007) Catheters Catheters are used in a number of procedures such as therapeutics, diagnostics, and ablative procedures. Used in the medical field for administration of fluids, drainage, and provide a method to insert surgical instruments, catheters are tubes that can be placed in a body cavity, vessel, or duct. In the case of blood vessels, the catheter must move around the bends and angles to reach the desired destination. Stiff materials would not be flexible enough for this procedure and may cause a rupture in the vessel. Due to heat restrictions and risk of damage, only specific shape memory alloys can be used for many of these delicate processes. A solution for this problem is provided by the R-phase transformation, which is a specific type of martensite transformation that occurs in certain nickel-rich Ni-Ti alloys (Langelaar, et al. 2010). Travelling through the vessels is a difficult task, so a steering mechanism is implemented into a catheter to maneuver throughout the body. Currently catheters are equipped with integrated micro-actuators that allow controlled bending, which yields enhanced maneuverability compared to conventional catheters. Actuators consist of guide wires that bend when energy runs through them such as an electric current that heat the shape memory material. The simplistic designs of the actuator allows for high strains and stresses needed for a process. There are few actuating mechanisms which produce more useful work per unit volume than nitinol (Williams, et al. 1999). Guiding wires also known as pull wires or shaping wires, are located along the tube to allow for motion in many directions. Above: This demonstrates that shape memory alloys are more effective in actuators than many of the current materials on the market. Guide wires provide flexibility, shape memory, and pseudoelasticity. When a greater stiffness is required, the thickness of the wire may be increased to meet performance standards. Shape memory alloys allow for the catheter to return to its original geometry when the tension in the wire is removed. One adaptation formed due to the lack of metallurgical joining is a stainless steel sleeve, known as a crimp sleeve, to hold the wires to the catheter (Stoeckel, 2010). The sleeve brings up the problem of increasing the diameter of the catheter. To prevent breakage in a material, more flexibility and ductility is ideal. In medical applications, nitinol has higher ductility allowing more plastic deformation without fracturing due to the temperature of the human body. At body temperature (310K), nitinol will have a high percentage of strain at low stress meaning more ductility. Stents One of the largest medical uses for shape memory alloys is in stents. A stent is a tube that is inserted into an artery to hold it open. Stents are needed when the walls of the artery are not strong enough to remain open and need support to ensure that blood is able to flow. The stent is put in place during a procedure called an angioplasty (Stent Facts, 2010). In order to get the stent into the artery, it needs to be collapsed and inserted into a catheter. Shape memory alloys allow doctors to collapse the stent to a much smaller diameter, and have it return to its original shape after leaving the catheter inside the artery. The original use of shape memory alloys in stents was in the form of a simple coil. The coil was tightly wound in the catheter and then expanded once it was inserted into the artery and warmed. The expanded size of the coil is chosen to be slightly larger than the inner diameter of the target vessel, which means the coil will not be able to fully expand inside th e artery. The shape memory alloy, in its warmed state, will continue to attempt to expand, which will put a continuous outward pressure on the walls of the artery. This will ensure that the artery remains open. In more recent times, simple coil stents are used more for non-vascular applications such as preventing bladder obstruction. The simple coil stents that are still in use today are used in vascular cases where easy retrieval is required. The shape memory alloy allows the stent to hold its form in the body, but still be easy to deform back to a straight wire for removal (Sutou, et al. 2006). More modern shape memory alloy stents are made in forms other than a coil. The shape memory alloy can be formed into a braided or knitted coil. The downside of this is that the points where the wires cross form thicker walls, which are undesirable in a stent. Although the braided and knitted shape memory alloy stents were a step up in functionality from the simple coils, the thicker walls made them undesirable for many cases. The next level of shape memory alloy stents occurred once scientists determined how to make the alloys in flat sheets rather than just wire. Laser cutting a pattern into a flat sheet of the alloy, then rolling and welding it at various points creates a stent with no overlapping wires at the walls. Sheet style stents are thin, but also structurally supportive when heated to body temperature. This gives them more flexibility than the simple coil models and is a better use of the shape memory alloys characteristics (Sutou, et al. 2006). If ingested gastrointestinal disorders are possible. Skin contact and eye contact include irritation with possible redness and pain. None of these side effects are chronic. (SMDS 2008) Complications of Nickel-Titanium in Medical Applications Of the wide range of alloys that contain the properties of shape memory alloys, nickel-titanium and copper-based alloys hold the most value commercially. Nickel-titanium, also known as nitinol, is an equi-atomic mixture of the two metals. Concerns have risen over this alloy for the fear of nickel being released into the body (Williams, et al. 1999). It is important in medical equipment for the materials to be biocompatible, or the ability of the material to perform with a necessary response. In most medical procedures no response is typically desired. To determine if nitinol meets these criteria, the properties of titanium, nickel, and the combination of the two can be looked at. Titanium is a metal with a high resistance to corrosion. It is not particularly reactive and therefore is effective for medical uses where the device needs to be in the human body for an extended period of time (Lagoudas, 2010). It contains no characteristics of toxicity. Titanium is also a very strong material, however it is rarer and more difficult to manufacture than other materials. This makes titanium expensive compared to other alternatives. Nickel is a metal which is extremely reactive. Nickel is toxic to the human body and may cause massive inflammation and interaction with proteins. These properties raise questions on whether nitinol alloy is safe for medical uses. The benefits of using nickel in medical devices is that nickel increases flexibility and lowers the expense when alloyed with more expensive materials such as titanium (Langelaar, et. al. 2010 ). The properties when nickel and titanium are alloyed together usually take on those of titanium. During the manufacturing process an outer layer of titanium oxide forms. Although some nickel will still exist on the exterior, the toxicity is greatly reduced. When choosing a material for medical instruments, a risk/benefit analysis controls which alloy will be used. Nitinol is chosen because it holds great benefits and is very safe to use. Extensive testing of this material has been done and is still occurring to limit complications (Yoshida, et al. 2010). Safety During Medical Application When considering the use of shape memory alloys (such as nitinol), in medical applications, it becomes necessary to evaluate the safety of the materials for use in the human body. Biocompatibility and corrosion are two factors that come into play when considering placement into humans. Properly treated nitinol implants are corrosion resistant and compatible in humans. These implants form a surface oxide layer that protects the base material from most corrosion. There are some concerns of the nickel content dissolving from the Nitinol and causing adverse affects. However, other alloys containing high levels of nickel, such as MP35N or 300 series stainless steel, have been used in orthodontics, orthopedics, and cardiovascular applications, all the while displaying good biocompatibility. (Stoeckel, et al. 2003) Studies have shown that in vitro dissolution of nitinol dental archwires in saliva released an average of 13.05 mg/day nickel. This number is significantly lower than the average dietary intake of 200-300 mg/day. There was no increase in the nickel blood level throughout the study. A comparative in vitro cell culture study was performed to measure nickel release from nitinol and 316L stainless steel in fibroblast and osteoblast cell culture media. The nickel content was higher in the nitinol group for the first day, but rapidly decreased over time to achieve similar levels as the stainless steel. The nickel content never reached toxic levels in the nitinol and did not interfere with the cell growth. It was found that samples prepared by mechanical polishing released higher amounts of Ni-ions than those prepared by electropolishing. In order to evaluate the effect of polishing on nickel release, mechanically polished and electropolished samples of nitinol, MP35N, and 316L stainless st eel were immersed in solution for a period of over 1000 hours. Samples prepared by electropolishing released smaller amounts of Ni-ions than those with mechanical polishing. The electropolishing process removes excess nickel from the surface and forms an enriched layer of titanium. (Stoechel, et al. 2003) A study on blood compatibility was conducted on nitinol and stainless steel stents using an ex vivo, AV-shunt porcine model. It was concluded that nitinol is significantly less thrombogenic than stainless steel, meaning that when used in the human body it has a much lower chance of causing blood clots. It is thought that the titanium-oxide rich surface layer on the nitinol prevents denaturation of fibrinogen and minimizes platelet-rich thrombus formation within the stent after implantation. (Thierry, et al. 2000) Comparison of Shape Memory Alloy Nickel-Titanium to Stainless Steel The ability of shape memory alloys to return to their original position after large strains are induced is similar to that of rubber. However, unlike rubber, shape memory alloys are strong and noncorrosive much like stainless steel. Both nickel-titanium and stainless steel have long fatigue life. Many stainless steels contain nickel to maintain an austenitic structure. Higher nickel content guarantees superior resistance to corrosive cracking. Stainless steel has a relatively lower cost compared to nitinol mainly due to larger production numbers. Only about two hundred tons were produced in 1998 compared to a few hundred thousand tons of stainless steel (Lagoudas, 2010). Alloying a metal raises the production expenditure but changes the tensile and shear strength of the initial metals. The properties of shape memory alloys are better than those of stainless steel and therefore are the chosen material for certain applications. Above: Shape memory alloys have two phases, each with a different crystal structure and  properties. One is the high temperature phase, called austenite, and the other is the low temperature phase, martensite. Each martensitic crystal formed can have a different orientation direction, called a variant. The assembly of martensitic variants can exist in two forms. Twinned martensite, which is formed by a combination of self-accommodated martensitic variants and detwinned or reoriented martensite in which a specific variant is dominant (Lagoudas, 2010). Costs of Shape Memory Alloys such as Nickel-Titanium Alloys such as nitinol have poor formability in the manufacturing process which increases the production costs of such materials. The complex behavior of the material makes the development of shape memory alloys adaptive structures a challenging task. In this case, it is generally accepted that systematic, model-based design approaches and design optimization techniques can be of great assistance (Langelaar et al. 2010). However, as more applications for these materials are needed, the price will decrease. Currently, shape memory alloys are commercially available from a limited number of producers. When more production of these alloys begins, production costs will reduce. World production is small in contrast to other metal commodities. Competition drives prices lower in a market. Newer technology in manufacturing will also make the production more effective. Prices for shape memory alloys were over one dollar per gram of material in the 1990s. Today, the costs are roughly ninety percent lower. Whatever the cost may be, shape memory alloys such as nickel-titanium are one of the only materials capable of such miniscule instrumentation with the desired properties. Shape memory alloys are effective for their cost due to reliability and multiple functions (Stoeckel, 2010). Many applications of shape memory alloys only require a small amount of material. With prices around that of similar steels, shape memory alloys are gaining more attention in a variety of applications. It indicates that CuZnAl is the best, while Ni-Ti is the least. However, it may be more advantageous to use Ni-Ti because of reduced voltage requirements due to much higher resistivity, which results in cheaper equipment in cyclic applications. Copper based alloys are less stable and more brittle than Ni-Ti. Although less expensive, copper based alloys have found little approval for applications. Future Trends Current studies at the University of OULU have been conducted in order to demonstrate that bone modeling can be controlled by using a functional implant such as a NiTi nail which can be used to bend a normal shaft of the long bone. The method could also be applied inversely, such as straightening a deformed bone. Fractures and especially frequent fractures lead to angular deformity and bowing of long bones. Operative treatment has usually consisted of cortical osteotomies with cast, internal fixation, or external fixation (Kujala, 2003). However, these are relatively large operations with much postoperative pain and a risk for complications. Implantation of a bending rod would be a much smaller operation for the patient with reduced postoperative recovery. It might even be possible to insert the nails using minimally invasive techniques which would require a minute incision. Thus, the functional nail presented might provide an easier, quicker, cheaper, and less painful way to correct such bone deformities in the future. In addition, Prototype piping in nuclear reactors has been wound with pre-stretched Ni-Ti wire, which leaves very high compressive stresses in the pipe. Tennis racket strings have been tested in China and the USA with both countries claiming performance superior to existing string materials (Deurig, 1995). Furthermore, a variety of damping applications are being examined including such motivated projects as railroad wheel tires and damping mechanisms for suspension bridges. Moreover, the maximum Ms temperature achieved in Ni-Ti binary alloys is 100 degrees Celsius and for several years scientists have searched extensively for ways to increase this. Ms temperature or Martensite start temperature is the temperature at which the transformation from austenite to martensite begins on cooling. Until just two years ago the only alloys showing hope were extremely expensive alloys such as Ti- Pd-Ni and Ti-Pt-Ni. Recently, however two new alloys are showing a great deal of promise, Ni-Ti-Hf and Ni-Ti-Zr31. These alloys prove that transformation temperatures of over 300 degrees C are possible (Deurig, 1995). However, it is too early to know what the cost of the alloys will be and if other properties will be as good as the original alloys. Luckily, these first indications seem positive. One advantage if such an Ms temperature is possible would include the use of nitinol in circuit breakers and in automotive applications. Conclusion Shape memory alloys are quickly becoming a common material used in medical applications today. The adverse uses of alloys, such as nitinol, allow for improved stents, catheters, bone plates, medical procedures, and more. These advanced materials are helping to shape medical technology for the future. Through their durability and unusual prowess for changing shape they have become the future of medical material.

Introduction This report seeks to examine the causes for resisting genetically modified (GM) food in the world. There have been resistance to genetically modified food have been going on since is commercial production began in early 1990’s (Glass-O'Shea, 2011). The European Union has been delaying decision to allow farmers to introduce GM food crops in their farms or importation of GM foods without labeling as compared to the North American counterparts. This report investigates the major causes of sustained resistance to GM food, the effects of this issue and possible recommendations to soften this position. Causes of resistance to genetically modified food There have been fears that genetically modified food could have far reaching health effects in after a prolonged time of consumption. Several studies indicated that other organisms are affected by the genetically modified food (Macek, Kotrba, Svatos, Novakova, Demnerova, & Mackova, 2008). Many leaders especially in developing countries are concern that genetically modified food is a ploy to enrich some certain corporations in developed countries. History of Public Health Systems Essay

Public Health is about helping everyone to stay healthy rather than focusing specifically on the individual, with the aim to promote health, protecting individuals from threats to their health and preventing ill-health. Public health policies have made a significant impact in increasing a person’s overall life expectancy and improving health. (Public Health) Public Health Strategies: Public health strategies are devised in order to prevent the spread of diseases, prolong life and promote health. This can be done through the use of monitoring, identifying, developing programmes etc. Monitoring the health status of the community – Is a key aspect of health strategies that are in place within the UK. This health strategy helps to monitor any changes that occur in the health of the population, along with alerting individuals to any potential problems. Health throughout the UK is monitored by quality of life, infant mortality rates and life expectancy (Baker L, 2008, BTEC National Health and Social Care Book 2, page 2) The monitoring of health throughout the country allows for advanced planning of local services within the community that may be at risk of certain health problems. The monitoring of health at a local level allows for information to be recorded before being compared to the health of other communities across the country. Local information on health is an important aspect as it is collected on a geographical basis throughout the UK, for example vaccination rates, hospital admissions etc. This is one way on health can be monitored, as in the cases of other communities they may have higher incidence rates of certain diseases whereas others may have low incidences of diseases. Communities that have a higher rate of disease are monitored further and health promotion campaigns will be developed before being put into effect in order to reduce the risk of disease spreading within the community. The health status of a community can vary throughout the nation and depend on a variety of factors, of which can include: Age Gender Socio-economic conditions Genetics Environmental factors Through the use of monitoring health changes any problems that may arise in the future within communities can be identified in advance in order for it to be prevented. For example the rise of sexually transmitted diseases within local communities would monitored in order to predict any potential problems that may occur in the future and stop them from taking place. Identifying the health needs of the population – The health of the nation is measured by using mortality and morbidity rates of which have indicated how people are now living longer than that of their predecessors. Identifying the health needs of the population is another important aspect of public health strategies within the UK; this takes place when trends and patterns in local communities across the nation are established. By identifying the health needs of individuals located in a particular community means that the need for services can therefore be identified. Patterns can be detected throughout the country through the use of national statistics. National statistics are used in order to determine how health can be improved and how areas of concern can be highlighted, along with the effects of ill health may be reduced and prevented. Patterns of illness and disease can possibly be the result of certain factors, of which include; genetics, environment, lifestyle, education etc. However some parts of the country may be more susceptible to certain illnesses and diseases than others due to the patterns that are outlined by the National Statistics and social trends information. Developing programmes to reduce risk and screen for early disease – ‘Screening is the process of identifying apparently healthy people who may be at increased risk of a disease or condition. They can then be offered information, further tests and appropriate treatment to reduce their risk and/or any complications arising from the disease or  condition.’ As defined by the UK National Screening Committee (UK NSC). Health programmes are developed based on the information gathered by epidemiologists. ‘An epidemiologist is a person who studies patterns of diseases or health risks in population groups, societies, and cultures.’ The Department of Health produces a green paper that proposes what the targets of health should be; and is based on these decisions as to how the government implement the findings. From this a white paper is produced, of which goes into detail as to how and what course of action is taken. An example of recent white papers can include ‘Our Healthy Nation’ and ‘Our Healthier Nation ‘ (1999). The aim of the white paper is to inform and protect members of the public by influencing social changes in regards to the health of the nation. (L, 2008, BTEC National Health and Social Care Book 2, page 4). Examples of current public health programmes include: Five a day campaign, MMR immunisation programme and the Local NHS Smoking Service. Controlling communicable disease – Controlling communicable diseases is an important aspect of public health strategies in the UK, of which ‘involves planning to include screening and early detection, isolation and treatment, containment, prevention and cure eradication where possible’. (Baker L, 2008, BTEC National Health and Social Care Book 2, page 7) The early detection of a disease can prove useful, as it can provide insight on the cause and the spread of the disease along with being able to highlight any potential risks that the disease may cause to an individual or group of people; especially in vulnerable people such as young children, and the elderly. Young children and the elderly are more susceptible to disease due to their immune system being much weaker than the average individual; therefore it is important in the early detection of an infection in order to stop this from occurring. Isolating individuals with communicable diseases enables to remain controlled, in order to reduce and prevent the risk of spreading the disease. A resident residing in a care home that was diagnosed with tuberculosis for example, would be moved to a room on their own, in order to ensure that the disease is not then passed to another service user within  the home is one example. Containing a disease can occur at a national and local level; this can occur once the source of the infection has been identified and plan has been developed and put in place in order to reduce the risk of the disease reoccurring. The measures that are taken in which to contain the disease can vary from short, medium, to long term measures. (Baker L, 2008, BTEC National Health and Social Care Book 2, page 7) Short term measures – limited visiting, unnecessary travel, treatment and isolation Medium/long term measures – immunisation programme, appropriate medical treatment, educating individuals about the risks, eradicating incidence of disease where it proves possible to do so Eradicating disease is now becoming possible due to advances in research and technology. Advances in technology means that more is known about the causes of illness and disease along with how the disease is spread. This advance has been made possible through the use of early detection and surveillance, monitoring, screening, treatment and immunisation programmes, health education and promotion. (Baker L, 2008, BTEC National Health and Social Care Book 2, page 8) Promote the health of the population – Health promoters are tasked with the promoting the health of the population on a local and national scale. Health promoters are based in a variety of settings, such as, GP surgeries, drop-in centres, radio, magazines and schools. They are prioritised on local need and the availability of funding for the necessary resources. These priorities can be identified through the number of reported illnesses and diseases through local statistics. Any illnesses or diseases taken from the statistics prove to be either life threatening or cause an individual to spend a substantial amount of time in hospital would then be given top priority in the promoting of health. Individuals who may be overweight can potentially be at risk of coronary heart disease later on in life. The health promoter would thereby ensure that diets and exercise are promoted through the use of proper channel on both a local and national level. Planning and evaluating the national provision of health and social care – The National provision of Health and Social care within the UK is planned and evaluated by the National Health Service and Social Services. This is based on the information provided by health and social care professionals on a local, regional and national scale across the UK. ‘The government have produced guidelines and information to state how they will tackle the problems controlling and preventing infectious disease spread.’ (Baker L, 2008, BTEC National Health and Social Care Book 2, page 8) The strategies outlined in the guidelines by the government are a series of proposed actions in order to create a system in which to prevent, investigate and control the threat of infectious diseases and to address health protection on a wider scale. M1 – Describe the origins of public health policy in the UK from the 19th century to the present day. In this essay I will compare two different health measures in the 19th century and how they have made an impact on society today. I am also going to compare and explain the living conditions of towns and cities in the 19th, 20th and 21st century. Public health has developed considerably over the years and the changes that have occurred overtime reflect on the health concerns of the nation during each time period. These changes are what have produced the Public Health system that is currently in place today. Over the year’s vast amounts of medical knowledge of today’s health professionals have increased in response as to how diseases are spread, along with the advances in medicine that have aided in helping to reduce the incidence of infectious diseases. During the 19th century the living conditions were exceedingly poor and there were various health issues of which include overcrowding in housing and overcrowding in general, thereby resulting in the spread of disease. The Poor Law Act (1834) was established in 1834. The Poor Law was designed in order to reduce the cost of looking after the poor and impose a system which  would be the same all across the country. The industrial revolution led to the development of towns and cities across the UK. The population of the nation had increased rapidly once the Poor Law Act was implemented. The country’s poverty relief system had not been amended since 1601, before finally coming into play as a result of Edwin Chadwick, John Snow. The Public Health Act was first implemented in 1848 in order to ensure that sanitary conditions were provided for in populated areas across the UK. In response to the Public Health Act 1848 The General Board of Health was developed so as to ensure that all public health policies that were administered were carried out as effectively as possible. Edwin Chadwick was the first commissioner of the board. The implementation of the act allowed for authorities working with civil engineers and medics to improve sanitation. (Baker L, 2008, BTEC National Health and Social Care Book 2, page 14) Edwin Chadwick (1800-1890) was accredited for his work on the reformation of the Poor Law. Edwin Chadwick was appointed by the government to carry out investigations and research into current sanitation. Chadwick wrote a report outlining his findings known as ‘The Sanitary Conditions of the Labouring Population’ published in 1842. In his report Chadwick argued that disease had a direct link to living conditions. After the report was produced new measures were taken in order to help promote the safe disposal of human waste and rubbish. Chadwick believed that poor sanitary conditions caused disease. John Snow (1813-1858) was a British physician commended for his work in relation to the cholera outbreak in 1854. Snow was an anaesthetist and epidemiologist interested in the practices of cleanliness and hygiene put in place to help prevent disease. Snow formed the link between the cholera outbreak to the contaminated water residing in the water pump in Broad Street. In 1854, Snow identified that a water pump in Broad Street located near one of the cess pits was what was contaminating the water; thereby linking the Broad Street pump as the outbreak site of the disease. Higher mortality rates were linked to the Broad Street pump. ‘He had the handle of  the pump removed, and cases of cholera immediately began to diminish.’ Throughout the 20th century more Acts and Reforms were put in place in order to reduce the risk of spreading infection that led to illness and disease among the UK population. This included; Beveridge Report (1942), NHS (1946), Black Report (1980), Acheson Report (1998), Our Health Nation (1997), Saving Lives: Our Healthier Nation (1999). William Beveridge (1879-1963) produced ‘The Beveridge Report’ in 1942, which went on to become the basis of a series of reforms after the Second World War, by looking into way on reducing inequalities in the health care provision. This report has been used as the foundation for most social legislation. The Beveridge Report was a major influence in the introduction of The Welfare State along with The National Health Service founded in 1949. The Beveridge report focused on sanitation and ways to improve and change the educational standards of areas which were highly affected by poverty. The report suggested that the working class gave a share of their wages in order to aid those who did not work, those of which who were either sick, unemployed, retired or widowed were then given these contributions. The government tasked Beveridge with the developing a report that was based on the ways that Britain should be rebuilt once the Second World War ended. The report was published in 1942 and provided recommendations to the government in order to find ways of tackling the five ‘Giant Evils’ known as ‘Want, Disease, Ignorance, Squalor and Idleness.’ Edwin Chadwick 1842, William Beveridge 1942 and the White Paper Report: making healthier choices easier attempted to improve overall public health. Whereas Chadwick focused on poverty, Beveridge focused on sanitation and the White Paper was specific to health. Beveridge and the White Paper Report: making healthier choices easier focused on poverty and how it affected a person’s overall health. Whereas in the Beveridge report he chose to write about The Welfare State, whilst the White Paper talked about reducing inequalities to health. John Snow and the White Paper Report both used statistics in order to provide evidence in order to give insight into the condition of public health and what need attention before giving suggestions on how to improve it. Christopher Columbus Fourth and Last New World Voyage

Christopher Columbus' Fourth and Last New World Voyage On May 11, 1502, Christopher Columbus set out on his fourth and final voyage to the New World. He had four ships, and his mission was to explore uncharted areas to the west of the Caribbean, hopefully finding a passage west to the Orient. Columbus did explore parts of southern Central America, but his ships, damaged by a hurricane and termites, fell apart while he was exploring. Columbus and his men were stranded on Jamaica for about a year before being rescued. They returned to Spain in late 1504. Before the Journey Much had happened since Columbus’  daring 1492 voyage of discovery. After that historic trip, Columbus was sent back to the New World to establish a colony. Although Columbus was a gifted sailor, he was a terrible administrator, and the colony he founded on Hispaniola turned against him. After his third trip, ​he was arrested and sent back to Spain in chains. Although he was quickly freed by the king and queen, his reputation was shot. Still, the crown agreed to finance one last voyage of discovery. Preparations With royal backing, Columbus soon found four seaworthy vessels: Capitana, Gallega, Vizcaà ­na, and Santiago de Palos. His brothers Diego and Bartholomew and his son Fernando signed on, as did some veterans of his earlier trips. Columbus himself was 51 and was beginning to become known around court for being eccentric. He believed that when the Spanish united the world under Christianity (which they would do quickly with gold and wealth from the New World) that the world would end. He also tended to dress like a simple barefoot friar, not like the wealthy man he had become. Hispaniola Columbus was not welcome on the island of Hispaniola, where too many of the settlers remembered his cruel and ineffective administration. Nevertheless, he went there after first visiting Martinique and Puerto Rico. He was hoping to exchange one of his ships (the Santiago de Palos) for a quicker one. While awaiting an answer, he sent word that a storm was approaching and that the new governor (Nicols de Ovando) should delay the fleet heading for Spain. The Hurricane Ovando forced Columbus to anchor his ships in a nearby estuary and ignored his advice, sending the fleet of 28 ships on to Spain. A tremendous hurricane sank 24 of them: three returned and only one- ironically, the one containing Columbus’  personal effects that he wished to send to Spain- arrived safely. A few miles away, Columbus’  ships were badly battered, but all of them remained afloat. Across the Caribbean Once the hurricane had passed, Columbus’ small fleet set out to look for a passage west. The storms continued, and the journey was a living hell. The ships, already damaged from the hurricane, took more abuse. Eventually, they reached Central America, anchoring off the coast of Honduras on an island that many believe to be Guanaja. There they repaired the ships and took on supplies. Native Encounters While exploring Central America, Columbus had an encounter many believe to be the first with one of the major inland civilizations. Columbus’ fleet found a trading vessel, a very long, wide canoe full of goods and traders believed to be Mayan from the Yucatan. The traders carried copper tools and weapons, swords made of wood and flint, textiles, and a certain beerlike beverage made from fermented corn. Columbus, oddly enough, decided not to investigate this interesting trading civilization: instead of turning north when he hit Central America, he headed south. Central America to Jamaica Columbus continued exploring to the south  along the coasts of present-day Nicaragua, Costa Rica, and Panama. He met several native cultures, observing maize being cultivated on terraces. They also saw stone structures. They traded for food and gold whenever possible. In early 1503, the ships began to fail. In addition to the battering they had taken from one hurricane and several major storms, it was discovered that they were infested with termites. Columbus reluctantly set sail for Santo Domingo and aid, but his ships only made it as far as Santa Gloria (St. Ann’s Bay), Jamaica. A Year on Jamaica The ships could go no further. Columbus and his men did what they could, breaking the ships apart to make shelters and fortifications. They made a peace with the local natives, who brought them food. Columbus was able to get word to Ovando of his predicament, but Ovando had neither the resources nor the inclination to help him. Columbus and his men languished on Jamaica for a year, surviving storms, mutinies, and an uneasy peace with the natives. Columbus, with the help of one of his books, impressed the natives by correctly predicting an eclipse. Finally, in June 1504, two ships finally arrived to pick them up. Importance of the Fourth Voyage Columbus returned to Spain to learn that his beloved Queen Isabel was dying. Without her support, Columbus would never return to the New World. He was getting on in years at any rate, and it is a wonder that he survived the disastrous fourth voyage. He died in 1506. Columbus’ Fourth Voyage is remarkable primarily for some new exploration, mostly along the coast of Central America. It is also of interest to historians, who value the descriptions of the native cultures encountered by Columbus’ small fleet, particularly those sections concerning the Mayan traders. Some of those who were along on the fourth voyage would later go on to greater things, such as Antonio de Alaminos, a cabin boy who would later rise to pilot and explore much of the western Caribbean. Columbus’ son Fernando would later write a biography of his famous father. The Fourth Voyage was a failure by almost any standard. Many of Columbus’ men died, the ships were lost, and no passage to the west was ever found. Columbus himself would never sail again. He died convinced that he had found Asia, even if most of Europe already accepted the fact that the Americas were an unknown â€Å"New World.† Still, the fourth voyage showed better than any other Columbus’ sailing skills, fortitude, and resilience attributes that allowed him to discover the Americas in the first place.
